CUTS Healthy Hearts

Cardiovascular Disease Prevention Project

The Healthy Hearts: Cardiovascular Disease Prevention Project is a collaborative Cardiovascular Disease (CVD) education and outreach strategy with LifeBridge Health’s Community Health & Wellness Ambulatory Services team to address the critical needs of marginalized uninsured and underinsured clients in Sandtown Winchester impacted by CVD and its comorbid conditions. The CUTS/LBH collaboration aims to utilize a multicomponent core mix of cardiovascular disease (CVD) service interventions designed to provide primary healthcare services and lifestyle resources to Sandtown-Winchester area residents. The overall goal of the CUTS/LBH CVD risk reduction initiative is to reduce or mitigate health disparities and advance health equity in the Sandtown-Winchester community.

CVD Program Services

Regular blood pressure screenings help identify hypertension early, providing residents with vital information to manage and prevent cardiovascular diseases.

Assistance with health insurance enrollment ensures that residents, especially the uninsured and underinsured, have access to necessary medical services and care.

A community assessment survey gathers feedback from Sandtown-Winchester residents to better understand local health needs and priorities, guiding effective outreach strategies.

Primary care referrals connect residents to local healthcare providers for ongoing medical care and management of chronic conditions like cardiovascular disease.

The Passport to Care program provides residents with valuable resources to access health services, ensuring they receive comprehensive care and support.

Community events and outreach initiatives bring educational materials, resources, and health screenings directly to the Sandtown-Winchester neighborhood, promoting wellness and preventive care.

The Community Health Expo offers residents the opportunity to engage with health experts, learn about cardiovascular disease prevention, and access resources for improving overall well-being.
